How to Complete IMM 5009 Application for Verification of Status

[vc_row][vc_column width=”2/3″ el_class=”faq_page_text_div”][vc_column_text]This page explains how you complete the IMM 5009 Application for Verification of Status (VOS) or Replacement of an Immigration Document form.

You should only use this form is you have lost your Canadian immigration document, or you want to fix a mistake on it.

The form can be used to replace temporary immigration documents, such as visitor visas, study permits and work permits. It can also be used to replace a Confirmation of Permanent Residence (COPR) or Record of Landing (IMM 1000).

When you submit this form, you will receive a Verification of Status (VOS) which shows your immigration status in Canada. It is usually delivered electronically as a PDF.

Completing Part A of the IMM 5009 – Personal Details

  1. 1. Client ID number/UCI: This is how IRCC recognizes you. You were assigned a client ID or UCI the first time you applied for status in Canada. It is an eight digit number.
  2. 2. Surname (Family Name) on entry: This is your last name, the last name you used when you landed in Canada.
  3. 3. Given Name(s) on entry: This is your first name and any middle names. If you have changed any of them, you need to enter the ones you used at the time of landing in Canada.
  4. 4. Current Surname: You should only complete this field if your name has changed since you landed in Canada, such as if you got married and took your husband’s last name.
  5. 5. Current given name: You should only complete this field, if you have changed your first name, or one or more of your middle names, since you landed in Canada.
  6. 6. Sex: Pick male or female.
  7. 7. Date of birth: Enter your date of birth. If you are coming from a country where records were not kept, use the birth date that was listed on your passport you used at the time of landing.
  8. 8. Place of Birth: Enter the city, region (if applicable) and country where you were born.
  9. 9. Citizenship: Enter your citizenship – if you’re Canadian now, you can enter ‘Canadian.’
  10. 10. Passport Number: Enter the number of your passport at landing and check “On Entry.” If you do not have your passport that you landed on, you can use a current passport, but know that there may be delays.
  11. 11. Date of Issue: You should be able to find your passport’s date of issue on your biography page, near the passport number.
  12. 12. Expiry Date: You should be able to find your passport’s expiry date on your biography page. It is normally 5 or 10 years from the date of issue.
  13. 13. Marital Status on entry: Enter your marital status at the time of landing in Canada. However, the second part of the question, “is your spouse a Canadian citizen or permanent resident?” refers to your current marital status. Confusing, we know.
  14. 14. Language of Correspondence: Canada is a bilingual country so you can receive correspondence in either English or French. You must choose one.
  15. 15. Current mailing address: This is where you want IRCC to send any mail correspondence, including your Verification Status (if you do not have email) or your replacement immigration document.
  16. 16. Residential address: If this is the same as your mailing address, click the box indicating so. However, if you live somewhere else – because your mailing address is your work, or because it’s a mailbox, or because you are using your Representative’s address, as examples – then you need to include the address of your physical residence in this space.
  17. 17. Telephone: If you think there is any reason IRCC may contact you by phone, enter your cell or mobile number here. If you are using a Representative, this should be the Representative’s number. Otherwise, list your home phone.
  18. 18. Alternate Telephone: Enter a second number here, such as your work phone number.
  19. 19. Email address: Your email is particularly important, as IRCC may ask you for additional information by email and you will be receiving your digital Verification of Status by email.

Completing Part B of the IMM 5009 – Documents Requested

  1. 1. What type of request are you making?: If you are replacing your Record of Landing, or your Confirmation of Permanent Residence, you are requesting a Verification of Status.
  2. 2. Immigration Document Date of Issue: If you are amending your document, or if you still have your passport that you landed on, you should have this date. If you do not have either the document or the passport, you may not be able to complete this section.
  3. 3. Canadian Port of Entry – Place of issue: This is where you landed. Example: Pearson Airport.
  4. 4. Indicate for which of the following you need a replacement copy or a Verification of Status document. If you have more than one valid temporary resident document, indicate the document ID number or if unknown, the document issue and expiry date of the requested document: First, select your document from the list. Then complete all information you know about the document. If you are amending, this information is on your document. If you lost the document, consult your passport at the time of landing for the issuance date.
  5. 5. Was your original immigration document lost/stolen/destroyed/other?: Select which option applies to you. Then, try to explain, in as much detail as possible, what happened to your document. If you are amending details on your document, check “other” and explain what you are amending.
  6. 6. If you are requesting a Verification of Status of an Immigrant Visa and Record of Landing/Confirmation of Permanent Residence have you applied for it before?: If you have applied for a Verification of Status before, indicate when.
  7. 7. If you are applying for a Verification of Status of an Immigrant Visa and Record of Landing/Confirmation of Permanent Residence and you are not a Canadian citizen, have you, since your admission to Canada as a permanent resident, been convicted of a crime or offence in Canada or elsewhere?: Answer this question truthfully. If you do not, it could cause problems later on.
  8. 8. Are you a Canadian Citizen?: If you have naturalized since you landed, indicate yes.
  9. 9. List all names you have ever used: If you have never used any other names, you can leave this section blank.
  10. 10. Did you enter Canada as Visitor/Student/Worker/Permanent Resident/Protected Person:
    • If you originally landed on a visitor visa (TRV), indicate visitor
    • If you originally entered Canada on a study permit, indicate Student
    • If you originally entered Canada on a work permit, indicate Worker
    • If you originally entered Canada as a permanent resident (emigrating as a skilled worker, or if you were sponsored outland, for example), then indicate Permanent Resident
    • And if you entered as a refugee, indicate Protected Person

Completing Part C of the IMM 5009

Most people do not have to answer these questions, as they are only for people who became permanent residents of Canada prior to 1973. So, if that statement doesn’t apply to you, leave this entire section blank.

Signing the Declaration on the IMM 5009

Sign and date the form. The date is the date you sign the form.
